Job summary

  • The Case Manager is part of a human service team that develops and executes service and care plans for residents to provide a supportive and empowering environment that fosters stabilization, independence and self-reliance, and skills development. The Case Manager plays a key role in executing and updating service and care plans, coordinating direct services and service referrals to external providers, and monitoring resident behaviors and practices.
  • Pay: $24.04 an hour- approx. $50,004 annually.
  • FLSA: Non-exempt. Eligible for overtime.
  • Schedule: Sunday - Thursday 9am-5pm (Friday & Saturday off)
